On February 14th, Lisa Smith received fatal gunshot wounds outside The Three Horseshoes pub located in Knockholt, Kent. The 43-year-old Slough resident was a mother at the time. The landlady, Michelle Thomas, described hearing two loud, noisy sounds, which at first she thought were fireworks. A large crowd totalling 50 people occupied The Three Horseshoes at this moment, resulting in widespread panic among the customers. Emergency services had not arrived before the off-duty firearms officer took charge of situational control until they came on the scene. Medical personnel delivered their response immediately but pronounced Ms. Smith deceased after arriving at the scene.

The authorities suspect the same person who fled from the scene next entered the River Thames after being acquainted with Ms. Smith. A vehicle and gun belonging to the suspect were located close to the Queen Elizabeth II Bridge crossing over the Dartford area. Detective Chief Inspector David Higham revealed that reports came in about a man who accidentally crossed the bridge barrier while police continued their searches in the river. Public safety is not threatened, according to authorities, because they target only one suspect.
